To take a RELATE Evaluation Questionnaire, simply follow the instructions below.

1. Select the RELATE package you want to purchase from the bottom of this page or go to http://www.connectioncare.com/compatibility-test
2. Fill out the required information with PayPal or with Google Checkout
3. Wait for an email from ConnectionCare.com with your token number – make sure you add to your white or safe list:mail@connectioncare.com
4. When you have your token number go to http://www.relate-institute.org/. In the “For Couples: RELATE” section of the screen you will click the “Start Now!” button
5. You must first create an account, so click on “Create account,” in red, to do so
6. Fill in you “User Name,” Password, etc. and then click “Sign up.”
7. Answer the question about your relationship and click “Begin”
8. Read the terms and click on “I Agree” if you do (Notice your progress to the left in the grey shaded area, under the RELATE institute icon)
9. Read the directions and click “Next” in the lower right
10. Enter other required information and click “Next”
11. You should begin the inventory with “Section A”
12. At the end of the questions you will be asked if you want to finalize your report. If you are ready to finalize click “Proceed”
13. Send a verification email to yourself. Make sure the email address in the box is the one you want to use. When you press “Send Verification Email:” a green notice will come up telling you that you should receive the email in 15 minutes
14. When you receive the email, click on “Verify” in the email
15. You will be taken to a screen where you can enter your “User Name” and Password. Do so and press “Sign in”
16. A screen should say “Your email has been successfully verified…” You can then click on “Proceed to Reports” in red
17. Click on “Purchase Report” in red
18. You will then click on the “Pay with token” button to select method of payment
19. Enter the token number email to you by ConnectionCare.com and click “Apply”
20. Another screen will ask you if we can view your report, please press “Yes” so that we can. All report are kept confidential and not given to any third parties or anyone else. If you still do not want a professional to view your report email us at mail@connectioncare.com and tell us, we will issue you a new token number that will only allow you and your partner to view the report
21. You can now view your report or add your partner if they have completed his/her report
22. To add your partner (remember you only have a certain number of days to do so) click on “Add you partner,” which is in red, on the report page
23. Where it says, “My partner’s username is:” enter your partner’s user name, of course
24. Enter a random fact about yourself that your partner would know and press the “Submit” button
25. Have your partner do steps 1 through 21
26. When you partner is done with the questionnaire and he/she has entered the token number, your partner will be taken to a page that says, “RELATE Report purchased.” Beneath that it will say, “Someone has requested to be paired with your evaluation.” And then, “Click here to approve or deny this request.” Click the link to approve the request.
27. You will be at the “Authorize a Partner” screen. You will click the button that says, “Yes, pair my results with this user.”
28. You can now view the report.
